Case Manager, Re-entry Program
Case Management is a key component of Gang Alternative, Inc.’s Re-entry Project. Case Management is a goal-oriented, process between the participants and the Case Managers from program enrollment through the follow-up period. Case Management includes administering multiple assessments including Level of Service/ Case Management Inventory, planning, coordinating, monitoring, and resource development.
The Case Manager will maintain responsibility for Navigational Case Management (tiered, triage formatted case management services) for participants of this project. The goal of this project is to help justice-involved adults (25+ years old) in high-crime, high-poverty communities get better jobs and thus, greater family self-sufficiency by reducing barriers to securing and retaining jobs that pay a livable wage. The expected outcomes are consistent with increasing employability skills and employment placement via improved workplace preparedness, attainment of industry-recognized credentials for in-demand occupations in Miami-Dade County, as well as reduced criminogenic risks through mentorship and navigational case management.
Essential Duties and Responsibilities
- Conduct outreach and execute recruitment processes consistent with the program goals and objectives.
- Conduct participant intakes, eligibility verification and documentation.
- Administer multiple evidence-based assessments including the Level of Service/ Case Management Inventory (LS/CMI), an assessment and case management tool that measures the risk and need factors of adult offenders.
- Utilize assessment results to inform the development of the participants’ Individual Services Strategy Plan (ISSP) which is the overall case plan that includes the agreed upon academic goals, career plan, employability skills, interest, and cultural background of each participant.
- Provide guidance and monitor individual participant activities as outlined in the ISSPs in order to track progress of individual participants toward attainment of identified educational and employment goals.
- Carry a coaching case load of participants continuing for a minimum of one year, including academic/career planning, goal setting, crisis management, and advocacy.
- Deliver evidence-based curricula such as those focused on integrated cognitive behavior-change programs for justice-involved adults.
- Provide case management services, training, academic guidance and mentorship to participants as needed.
- Accurately track, input data, and maintain comprehensive case files (physical and electronic) for participants.
- Facilitate group counseling in the form of education and skill building sessions on prescribed topics.
- Conduct frequent one-on-one participant sessions.
- Coordinate links to appropriate support services and provide appropriate referrals such as transportation assistance, basic needs and emergency assistance, academic tutoring, counseling, family health support and access to other support services.
- Prepare reports, as required, for submission to funder and other partners via Program Manager.
- Build and maintain key relationships within participants.
- Collaborate with other organizations and staff engaged in appropriate recruitment activities.
- Implement navigational case management with performance excellence and contract compliance.

Qualifications and Competencies
- Minimum Bachelor’s Degree in Social Work, Psychology or related degree. Master’s Degree in Social Work preferred.
- Minimum of three (3) years relevant experience in counseling and case management and/or providing training, mentoring, support to disadvantaged populations, in particular, justice-involved adults.
- Demonstrated experience in education and experience developing relationships with service providers.
- Ability to build and maintain relationships and partnerships; maintain confidentiality, professionalism and maturity in handling sensitive information, participant interaction and community relations.
- Knowledge of methods for job development, local labor market trends and employment opportunities.
- Demonstrated experience working with returning citizens providing counseling and assessment.
- Thorough knowledge of criminal justice field.
- Knowledge of case management principles, objectives, standards, and methods.
- Knowledge of Miami-Dade County community and government service delivery and case management delivery systems.
- Knowledge of motivational interviewing concepts and techniques.
- Strong leadership, organizational and analytical skills. Ability to organize, plan, and manage multiple tasks.
- Ability to manage time efficiently and meet deadlines.
- Excellent interpersonal skills; a collaborative, team-oriented approach.
- Ability to work with and engage individuals of diverse backgrounds.
- Exceptional written and oral communication skills.
- Must possess a proficiency working with Microsoft Office Suite and the ability to successfully navigate through the Internet, excel, power point, and other applicable software.
- Skill to communicate ideas and instructions clearly and concisely.
- Skill to coordinate with other staff, departments, officials, agencies, organizations, and the public.
- Other duties as assigned.
- This position will require occasional independent travel between headquarters and training and meeting venues. Must maintain valid Driver’s License and insurance coverage on personal vehicle at all times during employment.
- Performing the duties of this position requires the ability to walk, sit, stand, reach, talk, hear and lift presentation materials, including equipment.
