    Community Health Worker, SUPPORT

    Title: Community Health Worker, SUPPORT
    Reports to: HIV Program Coordinator, SUPPORT
    Employment Type: Full-time
    Work Location: Broward County
    Salary Range: $36-$38,000

    This position is directly responsible for assisting in providing HIV prevention services to underserved youth and adults ages 13- 24 years, under the agency’s SUPPORT program funded by SAMHSA.  The CHW will assist in engaging, testing and preventing the transmission of HIV and STIs to youth who are at high-risk of infection. CHW is responsible for providing case management services including: outreach and recruitment; screening; administering assessments; preparing client development plans; creating and maintaining client files; facilitating sessions; coordinating client development activities; monitoring and tracking client progress including administering data collection and evaluation instruments; and resource develop­ment including linking clients to appropriate support services.

    Essential duties and responsibilities

    • HIV prevention health education to community-school-faith-based at-risk youth/young adults
    • Assisting with community-based assessment and strategic planning
    • Outreach and community mobilization to engage high-risk youth
    • Assessment, case management/care cord., linkage with appropriate social/medical services
    • Health coaching and SUPPORT for HIV and STI testing and immunization services
    • HIV prevention rapid-testing and follow up referrals as necessary
    • Possible confirmatory laboratory testing
    • System navigation
    • One-to-one counseling using CLEAR
    • Life Skills training (BOTVIN)
    • Facilitating Red Ribbon and Hip-Hop 2 Prevent HIV and Substance Abuse EBPs.
    • Data entry.
    • Other duties as assigned.

    Qualifications and competencies

    • Position requires preferably a minimum of a BA in health education or health promotion or related or Florida Community Health Worker certification (related experience may be substituted for a BA degree on a year for year basis)
    • Minimum one-year experience as community health worker with HIV specialty
    • Post-hiring completion of agency OJT and CHW certification if not already certified
    • Excellent interpersonal skills, cultural competence /ability to work effectively with youth and adults
    • Fully bi-lingual English/Creole. Tri-lingual English/Creole/Spanish preferred.
    • Excellent knowledge of HIV health issues and conditions
    • Good communication and listening skills
    • Good organizational and service coordination skills
    • Good computer and data entry skills
    • Good knowledge of prevention and environmental strategies
    • Good facilitation skills, health coaching and training/ability to teach
    • Good health promotion education and advocacy skills
    • Data gathering skills


    • This position may require independent travel between work station and other sites. Consequently, the incumbent must maintain valid Driver’s License and insurance coverage on personal vehicle at all times during employment.
    • Performing the duties of this position requires the ability to walk, sit, stand, reach, talk, hear and lift presentation materials, including equipment.
