Salary Range: $45,000- $47,000 (non- negotiable)

Qualifications and Competencies 

  • Minimum of a bachelor’s degree in social work, and/or community planning or related field.
  • Bilingual Preferred (Spanish/English or Creole/ English)
  • Requires excellent leadership skills in community involvement and organizations.
  • Requires an unrelenting customer service approach.
  • Familiarity with the social work agencies in Broward County.
  • Excellent communication skills and ability to reach a variety of audiences.
  • Excellent organizational skills, dependent and responsive with attention to detail and ability to prioritize multiple tasks.
  • Requires excellent interpersonal skills, knowledge of the community and social services in the area, and resources for substance abuse, cultural competence, and ability to work effectively with youth and adult volunteers.
  • Proficiency in Microsoft Office Suite especially Word, PowerPoint, and Excel.
  • Excellent organizational skills, dependent and responsive with attention to detail and ability to prioritize multiple tasks.
  • Ability to work effectively in a team environment, excellent interpersonal skills with a positive attitude, and maximum discretion when handling sensitive and confidential information.
  • Willingness to work flexible schedules as deemed necessary including weekends and evenings.

Position Summary

Case Managers provide case management services and linkages to care for children, adolescents, and their families. Case managers also conduct outreach and recruitment; screening; administering assessments; preparing client development plans; creating and maintaining client files; facilitating sessions; coordinating client development activities; monitoring and tracking client progress including administering data collection and evaluation instruments; and resource development including linking clients to appropriate support services.

Essential Duties and Responsibilities 

  • Provide initial screenings and assessments that identify service needs and recommend options for care.
  • Provides and/or coordinates critical services for children and youth, which include, but are not limited to family stabilization, service coordination, system navigation, and facilitating access to both non-traditional and agency-based supports and services.
  • Ensures all clients are properly screened and assessed to evaluate clients’ appropriateness for program participation, maintaining written documentation detailing each client contact made.
  • Assesses client needs and family strengths and formulates an appropriate and responsive plan of action that addresses identified areas of need.
  • Conducts level-of-care determinations that have integrity and conform with agency and governance mandates, while concurrently ensuring that clients receive the information required towards making informed choices regarding program participation.
  • Ensures the maintenance of accurate, complete, timely and high-quality client records that comply with internal and external regulatory standards and agency policies and procedures.
  • Maintains accurate and confidential client records and contact information for all families involved.
  • Conducts case coordination in a strengths-based, family-centered, family-driven and culturally competent manner through a wraparound team-based process in which youth and families participate as partners.
  • Through the skills of brainstorming, assessment, negotiation, and consensus building, guides and assists the participants to produce an individualized course of action (ICA) plan that reflects the families’ strengths and incorporates GA or community resources.
  • Ensure open communication with partners who refer families and with programs who receive our referrals on progress or issues regarding the family.
  • Stays abreast of local and regional community issues, emerging trends, and best practices applicable to the work of the community service partnership.
  • Become familiar with and utilization of outside resources as needed to provide
  • comprehensive services to clients.
  • Keeps supervisor informed of day-to-day activities, significant events or conditions impacting clients and/or staff well-being, team safety, and office operations.
  • Serves as an Ambassador of Gang Alternative, Inc.
  • Other duties as assigned.
